 INSURANCE


In order to get your driver license back in the state of Ohio you will have to have Proof of Insurance Documentation.  That documentation must include the following:


1.    A policy of insurance in effect that meets the minimum standards under Section 4509.51 of the Ohio
       Revised Code as listed below:

·      $12,500 because of bodily injury to or death of 1 person in any one accident.
 
·      $25,000 because of bodily injury to or death of 2 or more persons in one accident.
 
·      $7,500 because of injury to property of others in any one accident.

·      Proof, to the satisfaction of the Registrar, that person is able to respond to damages in an amount at least
       equal to the above figures.


2.    A copy of the declaration page of a policy of liability insurance.

3.    Form BMV 2262, 2263 or 2294 completed by insurance agent (Back of Suspension Notice).

4.    A liability bond

5.    A financial responsibility identification card issued by insurance company.
 
6.    A binder of liability insurance bearing the original signature of an authorized agent and valid for no more
       than 30 days from date of issuance.

 
NOTE:     The policy must be written in the name of the person suspended.

 
       Anytime you are required to file Proof of Financial Responsibility Insurance wit the Ohio BMV, it can be done
       by one of the following:


·      Certificate of Insurance (Form SR22)[link to definition] – obtained from a licensed insurance agency

·      Surety Bond in the amount of $32,500 – obtained from a licensed insurance agency

·      BMV Certificate for money or government bonds in the amount of $30,000 on deposit with the State of
       Ohio Treasurer

       The insurance must be maintained for three years from the suspension date.
